Questionnaire COVID-19

Can you please fill in the questionnaire before coming to the Shiatsu practice.

If one of the answers to questions 1 till 5 is ‘yes’ then treatment is not possible, except for when a GGD Covid-19 test has been taken and the result is negative.

If you have filled in a ‘yes’ for question 6 then treatment might be possible after consultation. Can you in that case please call me?
